Community Members must complete the forms listed below in order to have access to the Whitman Wellness Center. You also must complete a training prior to using the facility. Once the membership form is completed, you may attend when the Wellness Center is open and one of our staff members will assist you.

•  We are always seeking anyone interested in assisting with supervision the Wellness Center.  Supervisors would handle check in to the facility, maintain a safe environment and ensure equipment is used correctly and also put away properly.  If you are interested in assisting with supervision of the Wildcat Wellness Center, Please start by filling out a volunteer application here and mail/email to  Paul Lahue, at plahue@mwcsd.org.
